PAC Code request in Lycamobile - what information is required?

Hi All,
This is my first post on this forum. I almost never write questions on my own, but this time I failed to find this information on the web.

I'm trying to move from Lycamobile to Three.
I requested PAC code via Lycamobile customer service form, and they responded on email, saying this is the required information:

"If you still wish to receive the PAC code, please help us with the below mentioned details.

SIM card number(19 digits number on the SIM card):
Frequently dialed number for the last 3 months is mandatory:
Date Of birth:
Complete address along with the postcode:
PUK number:
Last top up amount and date:
Network provider moving to:
Any particular reason why you want move from Lycamobile:"

My questions:
1. Why are they asking about frequently dialed number for the last 3 months? Is it some sort of validation?
2. Should I answer all of those questions?
3. I don't have my PUK number. Is this normally required by the operator during a move?
4. I have read that historically some Lycamobile numbers couldn't be transfered to other operators. I have read (but unable to link as a new user):
"Certain numbers which have historically been issued by Lyca are not standard UK mobile numbers, which could present a problem when transferring to another network."
Is this still a case?

Thanks

It's not normal - if you request it in person or via a phone and they have other means of making sure that you are who you say you are.

E-mail can be hacked and extra security questions are understandable as mobiles are used as an extra security measure by many banks and other organisations, and far too many scammers want to take over mobile numbers of their potential victims.

Not sure about the PUK. It's supplied together with the SIM.

We are sorry that you have chosen to leave us and would like to know why. Please contact Customer Services on 322 where we would like to hear your feedback or concerns. If you would still like to leave us, we can issue you the PAC code and you will be able to move to a different network.

They must either issue a PAC within 2 working days or a reason for why it cannot be issued, in accordance with Ofcom's "General Conditions of Entitlement".

It's almost certainly a security question, I'm aware some networks will won't issue a PAC from an email request because of the difficulties in verifying identity

Ad 3. Luckily I found my PUK code on their website, so that one is solved.

    There is some ambiguity here,

A PUK is a means to unlock a SIM card that has had the PIN entered incorrectly more than three times. It will not assist you in changing network.

A PAC is the code you require to give to a new network to move to them from your previous provider.

There is no any ambiguity. It's just extra bit of information that they want for security reasons before they issue the PAC.
