    Is your title shown on your birth certificate?

    In UK someone known as Major John Smith would be shown on documents as John Smith with his title of Major shown in place of Mr in the title box.

    Yep it is shown on birth cert

    Issues i have lived with for years is I have to keep telljng people what my name is

    I rather not have that two names and simply use one name across

    the Deed Poll website indicates that you have to agree to abandon your old name when you change to your new name.
