No, you just need to write the MR request letter. I would just concentrate on the MR. Go through my detailed post above (which took me quite a long time to construct), and make your MR as compelling as you can.
I don't feel the suggestion to ask for an explanation of why the report bears little relationship to what actually happened and is therefore totally inaccurate, will help you currently
You want to get the decision overturned asap - the route to go down is a written MR (and possibly your MP's intervention).
If you want to make a complaint about the assessment - the best time is after the decision has been reversed. Then, it's not just your opinion - you have been vindicated by a DWP decision maker or a tribunal judge. That's the time to complain.
Please, don't get sidetracked - concentrate on the MR.
I realise you must be very angry about the assessment - but you need to channel that anger into getting the decision overturned. My post and the links, show you how best to achieve that.

I once attended a pip tribunal with a friend who had also been in effect deceived by the assessor. She went from zero to over 20 points. At the end of the hearing i asked the judge and the two lay members, who included a doctor, what can be done about assessors whose reports are either misleading or untrue. Their reaction was that they felt sorry for my friend but did not offer to make a report to the dwp or say ATOS or Capita in order to bring the assessor to account. That does not seem fair. Claimants get sanctioned for all kinds of minor, or sometimes, non- infringements, yet assessors appear to be largely or wholly unaccountable, and get very well paid in return. Apologies for soap box rant, but trust you can appreciate my concern.0

Ok thanks. Odd that the tribunal don't refer you to such a service. That said, are most ICE reviews (as I would suspect they are) a whitewash ??0

Why would they? All organisations have complaints procedures and about 30 seconds of googling should find it.All shall be well, and all shall be well, and all manner of things shall be well.

In which he appears to have posed an entirely fictitious employment scenario, and then roundly abused any poster who tried to help him.

Therefore, I would suggest his comments really are best ignored - If forumites don't want unpleasantries directed at them, don't engage with this individual. You will only be wasting your time and effort.

I would also suggest reporting his unpleasant posts.
"Posters seeking advice in this forum need to bear in mind that this is a public site on which anyone can post. You can't assume information is trustworthy, indeed some posters are known to disrupt, post disinformation, and tell tall stories.
The MSE forum team do not moderate this forum.
I would caution forumites to be wary of any advice supplied that doesn't contain a link to an authoritative site. Such sites include adviceguide, entitled to, turn2us, revenuebenefits, gov..uk.
Accredited benefit advice is available from local advice agencies such as Citizens Advice.
Please don't take horror stories about the benefit process, assessments, appeals etc at face value. There are posters on here who whose aim seems to be to alarm, frighten, and generally dissuade OP's from pursuing sensible actions to claim / regain the right benefits.

Please kindly do not go over old ground Alice. I have in the past apologised and if I upset anybody I did not meant to. I was reacting to what I felt was undue criticism. One of my previous posts was not fictitious - it was largely based on true facts, but i had to protect the party for whom I was seeking answers. As I explained.
Anyhow, thank you for the feedback re the PIP complaints process. Almost 50% upheld, That's promising. Too late for the lady I helped to complain, but helpful for future reference.0
