Transfer a fixed ISA to Halifax

I have a fixed Lloyds ISA maturing in a few weeks time. I want to transfer the ISA to a Halifax Fixed ISA.
Is there an easy way to do this? I can't find any ISA transfer forms on the Halifax site.

Would there be a section for when the current fixed ISA matures?
In the past I have tried to carry out a transfer only for the new bank not to have a section on maturity so they in effect try to take the money out of the existing ISA straightaway.0 -
Just opened one. There is a question if you want the transfer to go ahead immediately or wait for the old one to mature. If its ready immediately halifax will back date interest to day of transfer request. Transfer must be done in 60days of opening.0
