Am I right in thinking this is for the cambridge diet? Seen a poster on the wall in my GP saying they provide a letter for this at a cost of £25.0

That's not the point - a GP can't sign to say that you will do because he/she can't control your behaviour.
Whoever it is who wants this signed should be getting you to make the commitment and sign the official paperwork.0 -
