Tick the second option. Always provide your details as per the NtK - otherwise it gives the PPC the excuse to ignore your appeal on the basis that you didn't follow their procedure.
They won't remind you of anything missing and will allow their appeal window (28 days) to close, and bang goes your opportunity to appeal to POPLA. Thereafter, that exposes you to up to 6 years of hassle, uncertainty and the possibility of a court case.
POPLA will kill this dead - don't miss it.Please note, we are not a legal advice forum. I personally don't get involved in critiquing court case Defences/Witness Statements, so unable to help on that front. Please don't ask. .

I have also just phoned the lease company to let them know I am appealing the fine.
the lease company also received the fine on the 8th of march (81 days)0 -
hey guys just a quick update to the fine;
"Dear Sir/Madam,
Thank you for your correspondence relating to your Parking Charge Notice (PCN).
Further to your representations, we have investigated your claims and can confirm that your Notice has subsequently been cancelled in full and no further action will be taken.
Yours faithfully,
CP Plus Ltd."
