    You mean to include with your ESA50? if so, yes good idea.

    Don't send the ESA50 to Medical Services yet.

    But you can send it or have it scanned to the ESA now if you want to.

    Finally getting somewhere I think.
    Rang the DWP on friday; they've finally accepted my reasons for not attending. I take it I'll need to fill out my ESA50 and get it scanned asap?

    Ok you could have already scanned your ESA50. but now do so as soon as possible. Alternatively, once you have it completed, you could phone Medical Services 0800 2888777 "help with questionaire" option. and ask for them to take the form from you over the phone, they will make an appointment to call you back...and then you dictate your answers to them over the phone.

    Or if scanning...
    Once it has been scanned, phone ESA the next day or better 2 days later,
    Confirm with ESA that your ESA50 has been received and sent to Medical Services.

    Then ask ESA to check your Medical Services contacts, ask them to check the address that Medical Services will be writing to and your mobile No. is correct with Medical Services. And to check there is no "Appointee/address" showing in medical services.

    Got my ESA50 scanned yesterday. I also got a call from the DWP to notify me that it had been received and would be sent to medical services. While I was at the jobcentre the person who scanned my form went through my file and made sure that all my contact details were up to date. I am not looking forward to hearing from medical services. I don't know what I'll do if they try to tell me I'm fit for work.

    Ok all good so far.

    Next week I would like you to phone ESA 0345 6088(?)... or Medical Services 0800 2888777

    And ask them to confirm your address and mobile number held in Medical Services (jobcentre will not have had access to this) And to confirm that there is no appointee showing in Medical Services. i.e. any post or text from medical services will be sent to you.

    While you're there ask them if there is a live referral in Medical Services
    and
    If there is an appointment for a medical assessment booked yet in Medical Services...

    I rang medical services. I have an appointment this coming Monday. Yay. Time to be judged by people who won't understand my condition and don't care if I question my existence every time I get up in the morning. If they try to bully me into working so they can save the tories a few quid that will be it. What can I possibly do now? My doctor point blank refuses to write me a letter. I'm stuck; snookered. They'll say I'm fit for work and I'll be forced to claim universal credit. I'll be destitute and homeless within a month.

    Worst case scenario, you can appeal and be paid on appeal.


    Looks like we have sorted this now, you have your appointment coming up and your claim did not get stopped.

    Well I suppose all I can do is wait and hope. I am not looking forward to it. My illnesses are mental, not physical, and all I've heard is that health services tend to treat people like me with open hostility and contempt. You'll know if all's gone to hell because I'll be back here.
