    I can't suddenly say to customers 'I need to charge you another £x as working for you makes me fall below the NMW'.

    Actually you can.

    Choose your time, perhaps 1st January and raise your prices for anything below your real NLW clients. Maybe structure the rises over 2 years to lessen the impact.

    IMO there is no reason anyone self employed finding their own clients should be earning below NMW.
