MBNA Promotional Rate

I've applied for a new MBNA All-round credit card with a 0% 30 month introductory rate for purchases, balance transfers and money transfers made within the first 60 days.
My question is, when does the promotional rate start? Is it from application acceptation or card activation? The reason I ask is that it's already been 2 weeks since my application was accepted and I've only received the PIN and direct debit details. Not the card. Have I lost 14 days of my 60 already?
