Where to sent/hand in a HC5(T) form

Hello good people of MSE land

As the title says. I know its not the Hospital. When i picked up the form the lady said Hospital no i said she then said the CCG.

But the form says sent it to Job Centre Plus she said no we dont have them here.

Any help/ideas folks ? Thanks

I've just looked at the form online HC5(T) and under Part 4 - Reason for Claim, there are details of where to send the form depending on your reason for the claim.

Thank you, i did point this out to the lady but she said i was wrong, as you say i will do what the form states. Thanks again
