Not really much point in mounting on the handlebars for accident recording purposes imo. You generally look towards the threat, so would be better placed on your helmet for that purpose. The only incidents you'll catch this way are ones that happen directly in front of you.

There has been much speculation but no conclusive proof that the GoPro did or did not contribute to the severity of his accident. It is known that the helmet broke and this is unusual.
IMHO, having a camera on a helmet will obviously increase the weight and consequently the force transmitted. It could catch on impact rather than slide as a smooth helmet is designed to do. It may result in concentration of the force of an impact onto a smaller area.
