The cheapest would be not to use the UK SIM card, but get a local one there. But this means having to give people a different phone number of course.
Perhaps your decision will depend on how many incoming and outgoing calls you expect. It would be worth checking BT mobile's tariffs for roaming calls; some companies have an international option you can take out for a month at a time for about £3, which cuts tariffs quite a bit.
You will need to use a phone that has different frequencies to here, triband or possibly even quad. Here we use 900 and 1800 bands. USA uses 850 and 1900. The Caribbean is something of a mixture; some copy US frequencies, others the 900/1800.
I've got my eye on a couple of new developments in US - new mvno SIMs coming soon.
