It may be worth asking the CAB if they have a benefits caseworker you could be referred to.

It costs £20 to subscribe, but the information is very helpful.
The first step in the appeal process is to ask the DWP for a "Mandatory Reconsideration" of their decision. As pmlindyloo suggests it is helpful to enclose medical evidence. Also if you have your original (11 points decision) send a copy of that. Describe how you still can't do the PIP activities relialbly, repeatedly, in reasonable time, and safely.
