    She can't get anything changed on the birth register but her father can apply for the birth to be re-registered and his name added. To do so he will either need the mother to go with him to do the re-registration jointly, or she can provide a statutory declaration confirming that he is the father.

    If there is no contact at all or prospect of her co-operating then he would need a court order - it's possible whatever happened in the 1990s may be enough (if he was given parental responsibility under the Children's Act 1989). He needs to find his paperwork or go back and insist on the court checking its records properly.
