As others have said, honesty is definitely the best policy- you should include it on your CV, especially if you want to continue to work in a similar field.
I know it's hard but you must accept responsibility for your actions. Even though you should have received more support from your managers, the allegations made against you are basic health & safety issues. You have put vulnerable people at risk and it's pretty clear that this job is not for you.
However, perhaps you should consider working in an advice/information setting (similar to your voluntary experience), rather than a practical care job. What skills have you gained from your voluntary work? Without knowing more about the voluntary roles I would imagine that you are a good listener and could work as a befriender/mentor. If you search 'Think Care Careers', there's a list of Job Types in Care work- hopefully you will find something that you can see yourself doing.

Street pastors are people from local churches who go into town centres late at night to support the drinkers is a short answerLost my soulmate so life is empty.
