Claiming BT Fibre £40 Boots card

I moved to BT’s infinity fibre last month having seen a deal listed on the Money saving Expert site.
I signed up on the last day of a deal that offered a discount and there were also links to pages to claim a £100 rewards card and a Boots £40 gift card once signed up.
Today I applied for the £100 rewards card which was really easy and only took a couple of clicks. The Boots card though appears a bit more complicated and I’m not sure I qualify!
You get asked if you signed up with BT.com and whether your service is up and running. A yes to each brings up a form to fill in where one of the questions is: what website did you see the promotion. I planned to enter Money Saving Expert but then in the terms it states the following:
A further condition states:
11 Once you have completed the form above, your details will be checked to ensure that your order was placed online and during the dates that the offer was running. If your service is active your gift card will be posted to the installation address. Please allow up to 30 days from your service being activated and claim being received for your gift card to arrive.
Has anyone successfully claimed the Boots gift card lately and is MSE a partner that just isn't listed?

Selected partners: 19th-30th June 2015, uSwitch: 14th – 28th August 2015, 8th-22nd October 2015, MoneySuperMarket: 28th August – 2nd September 2015, 15th January – 27th January 2016, VoucherCodes.co.uk 23rd October - 11th November 2015. - Only orders made online via designated websites and during the dates specified will be eligible to receive this offer.
