I am sure for WordPress there are loads of plugins for this type.
You could also you SurveryMonkey plugin, I think that can create an email field and sends out an autoemail/reply.
You can then set the file in auto mail message.

Install a plugin like Fast Secure Contact Form. When you design your form with it, make sure you include an opt-in for further contact.

If you're trying to set up something like an email newsletter, try the Mailpoet plugin. It has all the sign-up and unsubscribe options built in.

You can use an email service to do this. Most of them will offer way to do this but you have to subscribe to their service. I think Mailchimp offers this and there is a free and paid option so you if you use the former you can collect emails while sending out this factsheet.

Im just about to use Mailchimp myself.. free if you send to 2000 or less Subscribers

That Wordpress-compatible system may have a free option to do what you want.
Effectively it seems you want to deliver digital goods, I'm assuming at no cost but you do get the recipients email address. Best etiquette is to provide a tick box that the email address will be used for future marketing.
