After a bit of searching I managed to grab the last Samsonite cabin bag on offer in John Lewis. Down about £50 from RRP and I also had JL vouchers to put towards it anyway. Super light and very large capacity, plus external pockets for iPad etc..

Size wise it adheres to all the airline specs I usually fly with and comes with a 2 year guarantee as standard.

It's always in my trouser pocket because you never know what might go wrong. The obvious thing would be for the bag to get stolen but other things can go wrong...

And your pocket can be picked! We have a travel wallet holds everything important and yes my bag has been put in the hold but because what is important and expensive is in the front pockets they are easily retrieved before a case leaves my sight. :D

i am more likely to take things out my pocket when in the toilet and leave them behind or they fall out of pockets

You're right in that pockets can be picked but I'd suggest that it's far more likely that something would be taken from a bag than a pocket. It does obviously depend on what sort of trousers you're wearing. I generally wear jeans and the chances of being picked from my front pockets is minimal. Not impossible but unlikely. I do see that trousers with less tight pockets would be more of a risk, though.

I tend to wear a travel shirts that have internal zipped pockets

The Cabelas World Trekker is great for this and worth picking up if you're in the US, I have 3 or 4 now

Change is inevitable, except from a vending machine.

I travel a lot, without doubt the Eastpak Tranverz S is the best cabin case for the job. Amazon sell them for around £50 but you have to hunt as there are so many colours!
