If you read my thread on here it will give you an idea of what to expect.

The phone call is not a trap, you just need to confirm basic details like name, address and national insurance number.

The form just needs to be filled in with as many facts as possible.

The face to face is daunting but it's more of a chat than a medical and the person administering it doesn't do much of an examination.
