I phoned them and straight away they asked for debit card details i said wow hold on a minute she cant pay the full amount. They then said that ud need to fill out the form. Hope this isnt a ccj on her credit report0 -

Just spoke to them they said if paid within a month then she wont, but if she fills out the admission form to pay monthly then she will0

That is not a CCJ - it's a claim form.
The CCJ would not be issued until it goes through the court.
The options should be outlined with the pack -
Option to pay in installments would result in a CCJ being issued which would show on a credit record for 6 years.
The options would be
If you admit all the amount claimed and want time to pay - complete the admission form
If you admit part of the amount claimed - complete the admission form and the defense form
If you dispute the whole claim or wish to make a claim (a counterclaim) against the claimant - complete the defence form
If you need 28 days (rather than 14 days) from the courts date of service to prepare your defence, or wish to contest the courts juristiction - complete the acknowledgement of service
If you do nothing, judgement may be entered against you*
*if you pay the claimant in full then it should all stop
I've just noticed that it's been served 2 days in the future - perhaps this takes into account the delivery time (which they allow 5 days for).Starting Debt - £3082.10 - 02/01/16 / Current Debt - £951 - 5 Defaults satisfied

Fill in the court forms correctly, if you acknowledge service online you get 28 days instead of 14 to file your defence.
A county court judgement (CCJ) is the name given to the ruling the court will make, they are not automatically issued unless you fail to respond to the court paperwork.
If paid within 30 days, nothing further will be recorded, otherwise yes, it will be there for 6 years.
