Last pass. I have been using it for year and, with caution, trust it (they have had problems but the security model is safe in the event of compromise of their servers, with some caveats).

But if I were starting from scratch today I would be tempted to use iCloud Keychain as I live entirely in the apple ecosystem.

I use Keepass because it is available on all of the OSs that I use - Windows, Linux and Android. It's also Open Source so (in theory at least) it should be more secure than ones based on proprietary code (and of course it's also free).

I don't use any. I know this is going to bring scorn from some, but I have a small address book, which I have made alphabetized pages for in Open Office. I keep all website details and passwords in there and I hide it very well when not at home. I also have Bitdefender Total Security, which has a feature that allows the user to save all passwords then takes you straight to the site/whatever when the user accesses it.

Don't bother telling me how insecure this system is, it works for me and believe me, where I hide the address book is impossible to find.

I have a grandson who has eidetic memory and remembers every password on his own computer and phone. Sits at the keyboard, types them right in, no problem. Now, that's a useful talent!

I'd recommend LastPass. I recently tried several others to try them out, but came back to LastPass as the only one that worked the way I want.

    Ah, an actual physical book.

Well I suppose if you never get interrupted before you can hide it away, live on your own, never need to access any websites outside your house, have comparatively weak passwords that have no potentially ambiguous characters, and can guarantee you'll never have a fire, and don't mind it being far less secure than a password manager, I guess it'll do.

I use lastpass and the book. (you neve know). Statistically less likely to be burgled than cyber-mugged these days.
Since lastpass has been bought by logmein who are notorious for shafting their customers, it pays to be belt and braces. In fact, IMO, relying on one source is incredibly short sighted.
