The original question still stands (though perhaps the distinction between a transformer and a switched-mode PSU may be moot).
just to let you know, I got one of those 13 amp plugs with a usb outlet at 5 volts, and a usb to barrel connector for around £4, off ebay radios now working
I am pretty sure this has nothing to do with safety.
IMO, it's just simpler/cheaper for manufacturers to make identical devices with different external PSUs for different countries.
Like prowla above, I hate external PSUs, especially for monitors and printers (luckily, thing of the past I believe). IMO, the only justification for them is, indeed, safety, if the device can be used, say, in a bathroom, or portability - if the device can be used without PSU like, say, a laptop.
