There's also a direct Eurostar to Lyon which should help the connection issue.
So maybe possible with just one change?
Change is inevitable, except from a vending machine.

That would avoid a change in Paris, however AFAIK there is no direct train from Lyon to Milan. So either onto Zurich but I think a change is required in Geneva. Or possibly from Lyon to Chambery, change there and then onto to Milan.

The other thing that might be an issue is timetables - the direct London to Lyon trains are few and far between so whether there's sensible connection times from Lyon, I don't know.

There could well be other options with less changes though, but that will require someone with a better knowledge of European train travel than I have ;)

There appears to be a direct TGV from Lyon to Milan

So many options! Will have to have a sit down Tuesday night (1st September) when bookings open for Christmas and hopefully get something booked :)

I have just done a trip across France to pick up grandchildren, 1 child, 1 adult. The journey was about 700kms and for a return for me and one way for the grandchildren the total cost was £140.
    Much cheaper than UK!

How far exactly into your 3rd trimester will you be? Unless you're right at the very end it's perfectly safe to fly an a lot less stressful than such a convoluted train or car journey!

In your initial post you said you wanted a lie flat bed, unless you book an overnight sleeper service you're not going to get that. With the times and changes you're gong to be making the trains will be ordinary coach style seats, same as planes, but you'll be sat there for a lot longer than the 2hrs it takes to fly to Milan.
Accept your past without regret, handle your present with confidence and face your future without fear
