Sadly many use the NHS who have not paid into the system, EVER.
It's not a requirement.
The NHS isn't funded solely through income tax. There's also VAT on most goods, various duties on things like alcohol, cigarettes and fuel etc. A person can end up paying out quite a lot in tax in a year, even if they didn't pay income tax.
Your friend's neighbour might be best going to CAB or an age related charity for advice.
