info on low cost calls provider and i8866/1899

Please can someone help with a project i am doing on telephone usage.
I need to undrstand the difference between low cost calls providers (LCCP's) and intenet based 18866/1899 and how these work for a business switchboard system. Answers to any of the question below would really be appreciated...
1.what make a company and LCCP?
2Why is 18866/1899 so cheap - they say it is just low overheads but does it use a different carrier etc ?
3. What is an LCCP I know they have to contact BT to be 'carrier pre select' but why do some programme swithcboard to re-route calls instead?
4. If a comapny has a monopoly on the maintenance of a swithcboard and
then programmes it to route your calls though them at expensive rates is this legal?
3. why doesnt 18866 etc go via BT like LCCP's?
2. I have been told the call quality can be inferior using prefixes,
they are so cheap that they are getting to full capacity and you may not get through to your number andthat if you have a problem no one is there to sort it - is that all true?
3. Why do LCCP's contact BT to set up and 18866 does not?
4. Do LCCP's negotiate a deal with BT or just get permission to re route calls?
5. Is there asn advantage of using an LCCP and prefixes - in otherword how can the LCCP give you a better deal than using prefixes?
6. why don't LCCP's route calls through 18866 themselves - or do they?
7. What is so different about an ISDN 30 line regarding the above?
The problem understanding how the current phonecall companies work and what are your option if you get a new switchboard phone system?
By the way - i dont want to cover IP telephony yet - it is about a switchboard system..
