Not quite the same but I moved in to a new house last year and it took a while for the address systems to update and add our house name, trying to renew car insurance took an age because they couldn't find the address on their system even though I told them why. Computer says no!

A couple of true stories (well, they're on the web so they must be true). There is a company employee whose surname, Null, is blowing up their systems when used. Secondly, Ms Caterina Fake (co-founder of Flickr) stopped booking flights with Northwestern Airlines, as every one of her reservations was silently deleted by their booking system.
