Overall seems a pretty ok deal, I'm at work at the moment so can't exactly search to see if there are any better deals on this particular phone. (the poor camera is annoying though)
CPW give you 60 days to claim your chequeback (most offer 30) so very generous, don't think they ask for the original statement to be sent as they also accept online printouts. All in all not so bad (downside is the camera, you might want to go into a phoneshop and see the handset and how it works before hand)SLC: £13000+
