I'm a bit late in replying to you but you can get laser printable NCR paper, most office supply companies can supply it in pre collated sets of 2 or 3 parts. Once printed you can glue them usind special glue which also will seperate into the necessary 2 or 3 part sets. The paper works through tiny bubbles of ink on the back which burst when you press on them with a pen, so you could only print the basic form with a laser printer, you would need to fill in the information with a pen or dot matrix printer.
