Put both name and address but not your real signature as they DON'T need to see it. It's a letter, set it out like a letter (I said that to my very intelligent 19 yr old son just last week and he still needed to ask where the address went... they don't teach them how to write a snail mail letter any more!).

Your address, top right hand corner, miss a line, insert the date. Their address, left hand side a line or two under your address, miss a line or two ... Dear parking scammer, carriage return, I refuse to pay you because you are a bunch of bottom feeing scum.
