Suggest phoning their customer services - I also had little information from them and eventually phoned up to get my customer number so that I could login to their website. This is my back up SIM as costing nothing but therefore need to remember to claim the cashback. If you bought it from Mobiles.co.uk or E2save check your email (including Junk) for an email from them a few days after you signed up outlining the claim process and put the dates in your calendar ready to claim.
The DD details were taken when I signed up as part of the sign up process?
