Brand new Galaxy Note 4 smoking

My 3 week old Galaxy Note 4 has literally gone up in smoke. I came home from work and plugged it into the fast charger they supply and 10 seconds or so later my wife a I smelt burning.
We noticed smoke coming from the charging port of the Note 4 and I immediately pulled the charging lead out and gone a nasty burn on my finger for my trouble.
I have spoken to Samsung and they don't seem that interested but either I have been incredibly unlucky or these new handsets and their fast chargers are a fire risk, if I had left the room and this had been on something like paperwork it could have ended very differently.
I have spoken to Samsung and they weren't that bothered but if I am prepared to wait for a replacement they will send it away to be checked in a week or so! :T
The other thing I wanted to check with Samsung but couldn't get an answer was about the USB lead. Our Note 4 has a small white lead with the charger block and our Galaxy Tab 3 has a small white lead with the charger block, you know where this is going, if you mix the leads up do you get a problem?
Anyone heard of these fast chargers causing problems? What do you all think?

It was bought from a retailer. Samsung have asked me to send the Note 4 back directly to them so that they can look at it and I am doing so. They seem disinterested though.
My main concern is that I have a nasty blister healing on my finger and if that had been my little boy it could have hurt him a lot more. He uses my phone and both of the Tab 3's all the time and we have told him to always plug them into the charger if the battery gets low. Never crossed our minds that is might not be safe.
There were a number of S4's last year go up in smoke and Watchdog did indeed do an article on them.0 -

Why is it anytime something happens to someone the old "my child/fragile elderly relative" is brought into the whole story. I find this a lot in local papers. Picture of grumpy person, arms folded, looking like their partner has chucked them in for a younger model with the complaint lately based on "we have an 18 month old baby"0 -

Because when you are responsible for the care of someone, it's entirely reasonable to think about their care when something out of the ordinary happens.0 -
OP have you actually found out if mixing the charges up causes a problem yet (which I'm assuming it might due to the difference in output)?
OP is quick to judge Samsung and point out complaints on watchdog, oh the safety of my family is at risk etc... however it might be entirely the OP's fault for putting to much power into the battery!
Also, Samsung have offered to replace your phone from what you're saying. You might not be happy with the wait time for doing so but they've still offered to sort something out for you.
Unless you have some kind of business contract or coverage on the phone then 2/3 weeks is actually reasonable to get it diagnosed and replaced if needed. If you don't want to wait that long when things go wrong pay more and get a business contract0 -

I would suggest that you stop this immediately unless you want to pay for costly repairs to the charging port which would not be covered by your warranty.0 -

I'd imagine anyone that is that concerned about the well being of their young children wouldn't be letting them play with such devices in the first place.

The power was supplied by the 2A fast charger that ships in the box with the Note 4 so I am sure that is fine.
I am also perfectly comfortable with the USB lead situation. If I have used the USB lead from one of our Samsung Tab 3 devices then it shouldn't be a problem anyway but something cause it all the go horribly wrong.0 -
