Hi ... it's difficult to assist online as it would mean listing all your personal information, I would recommend contacting your local Citizens Advice Bureau who will assist in completion of the form or view their website for guidance. I think you have to complete a budget sheet showing all your income/expenditure which enables the court to reach a decision on affordable payments for you.
Sorry I can't be of any further help0 -

Over estimate all the other expenses by a fair and reasonable amount. Don't ever leave yourself short, budget for increases in all your other expenses. Make sure you pay all the current bills before allocating anything towards the arrears....including the water. Otherwise they'll just take you to court again for this years bill.
Evenly distribute your excess income, if any, towards arrears on all of your accounts based on the outstanding amounts. Include all of them including your council tax, gas, electric and water arrears.
Unsecured debt repayments are supposed to be prioritized lower than anything else so don't include them. They'll just say if you pay them less and you can pay us....if one of them is a credit card with some available credit limit they may ask that you pay the bill using your credit card to clear it.
Don't show a shortfall each month. Show you have at least £1 left over.:footie:Regular savers earn 6% interest (HSBC, First Direct, M&S)
Loans cost 2.9% per year (Nationwide) = FREE money.

Telling the truth would be the best (and only) advice I can give you.
If you still need more help, try taking the form with you to the CAB who will assist you in completing it correctly.0
