Not Divorced - Surname Query

    A mother's birth name should be on a child's birth certificate.

    If the mother is using her husband's surname, the mother's name box should say
    Mary Jones formerly Smith.

    If the mother has had previous marriages, it could say
    Mary Brown, formerly Jones, formerly Smith.

    If she has kept her birth name, it would just say
    Mary Smith.

    Ah thank you, I have just checked and yes it does say my maiden name, that's brilliant, one less thing!

    Would this affect me applying for a passport though, I was thinking as long as i've changed it all to my maiden name before I apply for it then everything should be ok? Also do I put Ms, Mrs, Miss on the passport application?!

    The mothers name, by which she is known at the time of the birth is what is shown on the birth certificate - not necessarily her own birth name. If she is married, then her maiden name (which is the name in which she married, again not necessarily her birth name) will be shown in the space for Maiden name.

    If she has had previous marriages or changes of name then they may be shown as "formerly known as" , or "otherwise known as ", but these can be left off if the mother doesn't want them recorded.

    The mother's name cannot be changed later unless there is found to have been an error and it has been wrongly recorded in the first place, or if an unmarried couple have children and then marry later , the children have to be re-registered and then the mothers married name can be shown.

    If your previous passport was in your maiden name, just apply for a renewal in that name. It doesn't matter whether you've changed other things.

    You can use whichever title you want - Ms, Mrs or Miss.

    The change in a birth certificate can be done when you get married - so your child's birth can be re-registered (although it's not compulsory) to show that their parents were married (legitimising them in old speak). It's not the same the other way round though, so it is your DS1's birth cert that could be reissued, not DS2's.

    In your case all you'd need to do to show your 'link' to the children would be to carry a copy of your marriage certificate.

    Also depending on the ages of your children security will likely just ask them, if they stop you, "Who is this lady?"
