Vodafone cancellation problems

Looking for advice on how to more forward after Vodafone nightmare!
in 2006 i started a business monthly contract with Vodafone, as I was working oversea a lot it worked out the cheapest option for me.
After starting with a new company and having a company phone, some time in 2009 I wrote to Vodafone to cancel the contact as stated in the contract i signed.
There was no conformation from Vodafone but on checking the phone the sim card no longer work.
I thought no more about this up until late 2010 when i started receiving statement from Vodafone paying for service charges plus vat of about £20 per month on an account i had asked by letter to Vodafone to cancel.
looking back over my bank statements there are monthly payments of £20 per month from the time the sim was cancelled.
after Writing to Vodafone with my company mobile number if they needed to contact me, visiting my local Vodafone shop and telephone calls to Vodafone to cancel the contract i stopped my direct debit to them August last year.
in no time at all i was contacted by Vodafone on my company phone regarding missed payment, I explained why I had not paid and asked again to cancel the contact and i would be happy to paid any admin cost for the cancellation, which i paid.
And again in December the same call the same conversation and again I payed.
as I have stated i work oversea and on my return in march i find a letter from Vodafone to contact them as soon as possible regarding late payment, which was a collection company again I payed and to my delight contract cancelled.
My delight has now turn to anger as i have just been refused a mortgage due to a bad debt listed by Vodafone.
I contacted Vodafone as soon as this became clear to be hung up on twice and after over two hours spoke to a chap called Sam who confirmed that i had been in contact with regards cancellation of the Vodafone account, but they had no record of any letters from myself.
I also asked them to correct my credit report which he would not do.
Should i contact trading standers or the governing body like Ofcom?
any help would be great,
