Hi there - I wonder if anyone can help regarding cases where the builder has been forced to fund or partially fund the cost to lay the infrastructure or upgrade a cabinet 'after the fact' where they couls have received funding from BT initially if they just would have sought it...

It has nothing to do with the developer. Openreach lay the cables.

What is often the case is that the survey to identify which cabinets will be supplied fibre connections is done years before the new developments materialise or the number of subscribers to the cabinet that feeds thenee development is too little to justify the cost of running the fibre. All this in spite of the fact that survey may have been done 18-24 months before any work commences.

Unless the developer deliberately bars OR from site, there is a national agreement between developers and OR....OR pay the developer to install OR's ducts, build boxes etc to OR's specification, if the developer doesn't want OR on site there will be a delay before OR can install lineplant in the footpath and roads , they have to wait until the roads and footpaths are adopted by the local council, and that could take years,
OR don't normally don't have to wait because of this agreement with the NHBC, if the developer is also a member of the NHBC then they get paid a set fee per house or per flat/apartment, the developer contacts the OR new sites group to get the ball rolling, obviously the best time to install duct, boxes etc is while the site is under construction, not after the road and footpath surfaces have been laid.
The builder isn't forced to do anything they don't want to, they are free to do an exclusive deal with another ( non OR) provider if they want, indeed there are places where the developer did a deal with another provider, and people who bought those property's have no choice to take service from that provider, even if they want BT or a company that uses OR infrastructure, like TT or Sky, they have no choice until the builder passes responsibility for the roads and paths to the council
Unless in a special designated area, where OR are installing fibre to the home, then new sites wouldn't be pre- fitted for fibre, but would get plain old copper pair service and would be retro fitted with FTTC/FTTH if and when that area was upgraded
