O2 can't take DD and showing on credit file

Hi All,
I am having an issue with my O2 contract phone.
There were a few issues to start with which have now been resolved, however they don't seem capable of taking the direct debit for my phone (the DD for the tariff is taken every month).
I have called them every month, been told every month that the DD is set up and there was 'an error' this time, however it is set up to go next time.
Lo and behold the next month rolls round and again they don't take the DD.
Every month I am getting sent default letters (currently the DD is 'taken' on a Sunday, so I have no way of checking it has gone until the Monday, by which time it is classed as late and a letter is automatically sent out to me.
My credit file shows the account as 'under review' which concrens me when I have made every effort to pay them, and they can't seem to be able to collect the DD.
Any ideas??
Yet again this month the payment is due to be taken on a Sunday, so guess I'll be seeing another letter *sigh*

Make a formal complaint, wait 8 weeks and if they don't sort their problem out go to CISAS. Amongst other things, demand that ensure and confirm that your credit files (external and internal) shows NO negative entries or this will be the least of your problems).0
