Flight Delay/Cancellation

Last year, I tried getting compensation for my mother-in-law for a flight she was delayed on back in 2012. The flight was booked through First Choice and should have originally been on Sunday afternoon going to mallorca. Instead, the flight was delayed or cancelled (nobody was informed what was happening), and they were offered the option to pay extra money to board another flight with a different airline, but at a cost. When she refused this, they were given a £10 voucher for food and drink and were then eventually put on a completely different flight over 8 hours later but this time flying with Monarch. We have written to various airlines using the template provided and we did have originally all the booking information e.g.. booking references. but have since lost all this. Since this has all come about again we are wanting to try and claim again but don't have all the info and cant work out whether we should be trying to claim for a cancelled flight or a delayed flight. Also when looking at the flight stats, we cannot find the original flight that should have left on the Sunday afternoon, we can only find the flight they did go on.

Please Help!!!
