'National Savings' slip found but no savings book

Hey,

I'm new here so sorry if I've put this in the wrong place :/

But yes anyway, when I was a baby my Mum set up a Post Office savings account (the slip I found says 'National Savings' with a little crown on the right) and I'm wondering how I'd go about getting the money from that account.

The only thing we can find to do with it is a green slip that says "New Account Customer's Receipt" but we seem to have lost the bank book when we moved house a few years back.

Annoyingly my last name is spelled wrong on this slip ('or' rather than 'er' on the end of my last name - the slip is hand written though so the account might have everything done correctly.) My address is also different (old address).

Anyway, I'm just wondering how I can go about getting the money from this account. I found a form on NS&I's website for claiming stuff but I don't know if that would be any use considering I don't have any account numbers.

Can anybody give any advice?
