The entry process can vary depending on which country you departed from, which passport you hold and if your are resident in another country.
It provides a much better overview of the specific entry requirements, I hold dual citizenship and there countries out there that don't recognise dual citizenship, the Qantas checker warns you in advance and also notes other entry requirements which are often overlooked on Government websites.
I don't see the requirement to hold onward tickets mentioned on the Gov site, but if you turn up in Malaysia without a printout showing your return/onward ticket it may cause problems when trying to enter the country.
It also provides advice as to which vaccination certificates you need to enter the country.
The airlines use the same system to check you can board the flight, they face hefty fines for allowing passengers to fly without the correct documents, visas, letters of invitation, vaccination certificates etc, so you can be assured the information is up to date

It will be a new passport number, she can in theory get the new passport in either name however whichever she goes for it'll need to match the airline tickets and there is no option to change the passport short of buying a whole new passport again.
Whilst it is unlikely to happen, naturally carrying documents/ credit cards in a mix of names is never going to look to good, especially recently issued ones, to border control officers if they decide to get shirty0
