Take it your landline has never gone down, if not I advice you to check your service agreement. Most if not all take a few days to repair and you do not under law have a right to a temp phone.
I'm not saying whether or not one would be entitled to a temporary phone, but whether this is significant inconvenience. A reasonable person would say this is significant inconvenience. Furthermore when one's fixed line goes down, BT does divert one's number to a mobile number, the cost of which BT covers. This is equivalent to a temporary phone.

When dealing with iPhones, Orange/EE's main concern is typically not causing any inconvenience to themselves, not to their customers.
They 'say' it's dictated by Apple, because Apple control over their own products

It's their own internal business, not customers'. Apple can dictate to Orange, they can ignore the law, but neither can change it.

OK we finally go this solved.

Orange gave my wife an oldish phone for free to keep while it got sorted. We wont go into the nano vs normal sim card issues we had.

She had to take her iphone to the apple store. The customer assistant fiddled with it and went out the back then came back out said its faulty we will replace it . The manager then stepped in and said no come back in 30 mins we need to take it out the back again.

My wife went off for a look around which i was concerned about as to be honest I had visions of them trying to accuse us of getting water in it or damaging it.or trying to get out of replacing it.

She went back in and they agreed to exchange it (which was probably with a previously repaired one as it didnt come out of its own brand new box with leads , chargers etc.

We have incurred the cost of the train fair and been without any phone for 3 days for her and without the iphone for a week before it was sorted. Pretty unsatisfactory really.

Orange have agreed to give us £20 off our next bill for the trouble. Apple will be getting a letter but I expect they wont even read it.

O2 shops are giving out free nano-SIM adapters. When micro-SIMs were unusual, Orange used to give out free adapters with every micro-SIM, but they stopped doing this when nano-SIMs were launched.
