Error on CPW Chequeback Claim Form

Has anyone come across the error on the following CPW claim form?

I'm about to make my first claim on a 12 month contract and there are 3 claims to be made by sending 4th 8th and 12th airtime bill with the appropriate coupon.
The coupons say;
1st Claim - 4th bill
2nd Claim - 4th bill
3rd Claim - 8th bill
4th Claim - 12th bill
Should I just ignore this error and, when the time comes, just send coupons 1, 2 and 3 with bills 4, 8 and 12?

You don't actually need to send off the coupons to get your cheque, as many other people here will also tell you the same.
The incorrectly printed coupons will probably just confuse the guys processing your cheque so I'd bin them or use them for origami

I sent my 4th airtime recently. This is the first time Ive ever done a cashback scheme as Ive always been wary of them.
All I did was print my invoice from the website then sent that to the right address. I believe there are different addresses depending on what provider you are with.
I then got a txt message from them saying they have received my bill and are processing the chequeback.
When u signed up, u would of got a welcome pack which describes what you need to do.
If u are worried then a simple phone call to CPW will help. I found them to be quite helpful.:hello:

Shopping Tips
Don't spend five pounds to dry clean a shirt. Donate it to the charity shop instead. They'll clean it and put it on a hanger. Next morning buy it back for two pounds.0 -
yup cpw dont need the coupons/vouchers/certificates... just include a short covering letter with the number your claiming for...ummm...0
