    I am guessing this is to do with readability by blind people.

    Some of these PDF creators work by turning the page into an image and embedding that into a PDF. Other PDF creators work by storing the actual text and saying "use this font, and size and put the text here".

    Only the latter can be picked up and understood by screen reader software, so if you use the former blind people cannot read your document.

    If you install Acrobat then you get that option anyway.
    Not sure if you need the full Acrobat or just the reader though.

    Exactly this.

    If you use the save as pdf option in recent versions of Word, then so long as you format your document properly using styles, the resulting pdf will be easier for blind people to use through screen reader software.
