Completion of Mat1B form...

I'm not due to see the midwife again until 25 weeks+ which would be too late for the mat1b form, plus i want to get it in sooner than that as i don't trust HR!

I've called my doctors to ask if I needed to book an appointment to get it filled in and the lady said she's 'pretty sure' i can just go and pick up a form, do my bit and hand it back in and the midwife will fill it in next time she's in.

I'm probably just worrying over nothing but is this right? just i'm sure i've seen the form online somewhere and it says 'i examined this patient today...' so do they need to see me? presumably they have a copy of my notes anyway?

  • I'm not sure about that either. Can't remember how it was last time. But this time when I went to my 12 week scan they said they'll send it out in the post but that was 2 weeks ago.

  • I asked my midwife for one, she left it with reception and I collected it.

    You can get it from 20wks onwards.
    If you aim for the moon if you miss at least you will land among the stars!

    Can def get midwife to complete earlier than 25 weeks. Mine needed to be in before the 25 week deadline at work. It's just a small half A4 paper size stating EDD and midwifes siggy. Took mw 2 mins to complete.

  • Get your midwife to feel it out and either leave at the surgery or possible post to you???

    My surgerys reception said the same thing so I went there only to be told they don't keep them as only midwives have them.
