PPI letter and complaint form from Natwest

Hello All,
I just received a letter from Natwest in regards to PPI. First they explain what PPI is and how they have concerns it was sold to some customers. Then they provide info about loan accounts I had with them. That is followed by a form as below.
I do not remember ever agreeing to purchase PPI unless it was made clear to me that it would help to process the loan application.
Please advice, should I respond on the attached form or rather use a template provided on MSE.

Use the form they supply.
You have just received a standard CCL. It doesnt indicate any issues. It is just an invitation to complain if you feel you have reasons for complaint. It allows them to start the timebar clock. I'm sure what sure I write on that form under B.3.
I think what applies to me is that I did not understand that PPI policies were optional and in some cases I felt like it would help to process the loan application. Any suggestions what should I write under this and other fields? Sill confused...

Nobody can tell you what you should complain about, every case is different. Just answer the questions truthfully, they may have data showing how it was sold/ how you bought it at the time, if you fabricate your answers you'll look like you're trying it on and lose all credibility.Any suggestions what should I write under this and other fields?0
