Injunction against my neighbor for harassment

Hi all,
I am harassed and threaten since several months by my neighbor and I have been advised to file an injunction form against him. My local citizen advice bureau is closed for a week but I am really in emergency.
Also, I am looking for a sample of the injunction form (N16A) filed for harassment.

Ps : I am presently having difficulties in filing this :
"The grounds of this application are set out in the written evidence of(6) sworn (signed) on" since my application will only based on my witness statement and proofs (police records).

Harrassment is a criminal offence, have you reported it to the police? That they got your statement, other's witness statements? has your neighbour been arrested?0

Police told me to send them to the court.0

If you want proper advice, i/we need more information.
You need evidence, police reports, witness statement, council complaints statements.
The police cannot ignore harrasment, which is a criminal amtter. while this injunction is a civil matter.0 -
All the rest are done : now I just have to fill the injunction form.0

I'm sorry i can't help without knowing more about this.
Remember this is going to cost you money. and it's not guaranteed you'd get it. I seriously hope that whatever has happened is serious, because you could end up paying alot for nothing.
The police do not do civil law (and this is a livil law case), you would genuinely get better information from us with more details.

The main purpose of my thread is how to fill this following field of the injunction form :

Can i just add, police records are not necessarily proof.
Judges, like to have witnesses. Documents only go so far.
Anyway, is (6) not you?0 -
I have equally my witness statement and proofs like emails by which I was equally harassed and threaten.
Emails?! EEEKKK
How can you prove that it was the defendant that sent them?
Emails and text messages are really difficult to prove, letters abit easier with hand writing experts etc.
Remember this is a court, they want good evidence, as they are lmiting the freedom of movement this individual will have.
Was this individual ever arrested? If so you could call the arresting officer to court.
Is there any CCTV of any of these incidents?
Is this person related to you, an ex, or a neighbour? (if not you'll struggle even more)0 -
He is a tenant in a sharing house related to my landlord and this last is equally evolved in my harassment and was using the same email address for more than a year (it is the landlord who was threatening me by email ).Emails?! EEEKKK
