SKY Complaint Address

Can someone please supply a name and an email address/postal address for a senior person at SKY to deal with a complaint, as I'm having no luck with the Complaints dept..
How do I word this email/letter and what time constraint is there for them?
My problem
I was lured to SKY from O2 by the offer -
EITHER
Free SKY TV and broadband for a month for the first year
OR
Broadband only @ £2.50 per month for the first year
When I called I was talked into the free TV and broadband deal, but phoned up 2 days later and said we watch very little TV, so this is pointless and I'm happy to pay £2.50/month for broadband only. There was no argument about this.
When I received my first bill I was charged the full amount for broadband. I queried this and was told that when I cancelled the free TV and Broadband deal, the £2.50 broadband deal was not available. I was not told this at the time.
I have the name of the person I spoke to and the time of the phone call. The emails have been going back and forth without progress. The latest one said they'd reduce the broadband charge by half for a year and staff training has been revised. Good bye.
Still the full amount was debited from my bank account.
I've sent a letter but no reply.

is the chief executive address, it doesn't go to him but an escalated team who will be able to help.
I don't know why you're declining free sky for a year as you at least get a freesat-from-Sky setup which is £175 otherwise.0
