It says:

"CANCELLATION
11. Relocation: This agreement can be cancelled in the event that your new permanent address is more than 15 miles away from the facility
upon receipt of a copy utility bill or bank statement showing the new address"

Did you ever have to send in a utility bill/bank statement? Anything you sent in writing would assist your case.

Yes matttye! that is the gym. It has been a long while so I don't remember exactly but I definitely don't have anything from them in writing neither I was asked to provide the proof of new address. However I do have letters in my name on the address I moved to covering the period when the default was registered.
