    Hi - bullet points

I got a replacement 3GS under warranty from Apple Store.

    My Q is :-
How and when did the lock process occur? If it helps, the original broken 3GS was not a Tesco phone either. So the only link to Tesco were the contract SIM card in each case.

Briefly, the warranty replacement would have the same lock as the original, it's programmed in as part of the warranty replacement process.

The original handset would be locked at original point of sale, if you bought it on contract it was locked when you got it. It would never lock at any other time.

If you used the phone for a long time period on Tesco they might be amenable to your unlocking request, they won't unlock for someone who hasn't been a customer. If they won't help you (if you don't meet their criteria for unlocking), then the original contract owner would have to request it.

Why? The phone has gone already. Clearly?

Edit: I made an error when advertising the phone. I thought my original post made that pretty obvious? Anyhoo ....

No, you didn't really make an error in advertising, at least not knowingly. Your error was failing to establish the lock/unlock status. My point was that a simple SIM test would have avoided all the hassle you now have in taking the phone back and refunding the buyer, since it doesn't sound like you'll necessarily be able to get Tesco to unlock it.
My comment might not help you now, but it might help others from making the same error.
