I've had numerous contracts with CPW, never bothered with the form and they have never ever cared. I don't think even they know whether its worth bothering with or not as they hardly ever send it out by post apart from when they first started using them.

I agree - CPW doesn't care if you send off your bills without a cashback form. If you go into a shop to ask for one, they'll tell you that you don't need it
