PhoneBoxDirect - first cashback claim - yearly amount?

Hey everyone,
In January I signed up for a 12 month free deal on a 12 month contract with PhoneBoxDirect. It's now time to make the first claim so I'd like anyone with cashback claim experience with phoneboxdirect to check the following. I still have my order number so that part is fine as far as the form goes.
1. Yearly cashback amount. I don't know how much this was and never actually received an email with the details in but can remember it being four hundred and something. It's a £45/month package with 3 (1100 xnet) and the first three months are half price as follows:
Jan £22.50
Feb £22.50
Mar £22.50
Apr £45.00
May £45.00
Jun £45.00
Jul £45.00
Aug £45.00
Sep £45.00
Oct £45.00
Nov £45.00
Dec £45.00
Total = £472.50
So £472.50 is what I'm going to put in the yearly cashback amount on the form. Can anyone advise if this is correct? It is how their other cashback deals seem work out. I shouldn't minus £2.50 due to an admin fee somewhere or something?
2. Claim amount. The form says total divided by five which would be £94.50. I'm confused that they are saying five equal installments when this is after four months and the rest are after two months. Can anyone reassure me that yes it is just the total divided by five?
3. What to post. I have my month 4 bill and my first bill (due to porting). Along with the claim form, is that all I need to send for my first claim?
As mentioned, I'd really like to hear from anyone who's done successful cashback claims with phoneboxdirect to check all the above is right as I'm guessing any minor error and that will be my cashbacks shafted!
